SleepTestKit provides a streamlined, clinician-directed platform designed to facilitate the evaluation of sleep-related conditions and access to home sleep testing.

Through the Website, SleepTestKit:

  • Collects and processes patient-submitted health information

  • Facilitates a structured, clinician-reviewed eligibility process for home sleep testing

  • Coordinates the delivery and use of home sleep diagnostic devices

  • Ensures that sleep study data is reviewed and interpreted by licensed, board-certified sleep specialists

  • Facilitates access to treatment pathways, including referrals to third-party durable medical equipment (DME) providers where appropriate

SleepTestKit may utilize various FDA-cleared home sleep testing technologies to support data collection and analysis.

2.2 Clinician-Directed Process

  • SleepTestKit operates through a clinician-directed workflow.

  • Patient-submitted information is reviewed by licensed healthcare providers, who determine, in their independent clinical judgment, whether home sleep testing is appropriate. Where appropriate, such providers authorize testing and interpret diagnostic results.

  • SleepTestKit coordinates the administrative, technical, and logistical aspects of this process but does not itself provide medical diagnoses or treatment.

2.3 Scope of Services

SleepTestKit is designed to:

  • Improve access to sleep health evaluation

  • Streamline the process of obtaining and completing home sleep testing

  • Facilitate efficient review and interpretation of diagnostic data

  • Support coordination of follow-up care resources

SleepTestKit does not provide emergency medical services. If you believe you are experiencing a medical emergency, call 911 immediately.

2.4 Limitations of Services

SleepTestKit:

  • Does not independently diagnose medical conditions

  • Does not independently prescribe medications, diagnostic tests, or medical devices

  • Does not replace a patient’s relationship with their primary care provider or other treating physicians

All clinical determinations, including authorization of testing and interpretation of results, are made by licensed healthcare providers exercising independent medical judgment.

2.5 Third-Party Services

  • SleepTestKit may facilitate access to third-party providers, including durable medical equipment suppliers and other healthcare services.

  • SleepTestKit does not manufacture medical devices and does not control the clinical decisions, products, or services provided by third parties.

2.6 No Formal Clinical Consultation

  • SleepTestKit’s services do not include a comprehensive medical evaluation or a formal evaluation and management (E/M) visit, whether in-person or via telehealth.

  • The clinician-directed review of patient-submitted information is limited in scope to determining the appropriateness of home sleep testing and interpreting diagnostic results.

  • This process does not replace a full clinical consultation with a licensed healthcare provider. Patients are encouraged to seek additional medical evaluation for ongoing care, diagnosis, or treatment as needed.

 

 

3. No Medical or Physician-Patient Relationship

Use of the Website does not create a physician-patient relationship between you and SleepTestKit.

Any medical services are provided solely by independent licensed healthcare providers. Any physician-patient relationship is strictly between you and the provider you choose to engage.

 

4. Independent Providers

SleepTestKit facilitates connections with independent healthcare providers (“Providers”), but:

  • Providers are not employees or agents of SleepTestKit

  • SleepTestKit does not control or influence clinical decisions

  • SleepTestKit does not guarantee the quality, appropriateness, or outcomes of care

  • Providers are solely responsible for their services, licensing, and compliance

Payments for Provider services may be processed through the platform but are for the benefit of the Provider.

Reimbursement is not guaranteed, and you are solely responsible for verifying coverage, eligibility, and reimbursement requirements with your insurance provider.

7.3 No Guarantee of Coverage

  • Insurance reimbursement

  • Coverage eligibility

  • Amounts reimbursed

Any reimbursement decisions are made solely by your insurance provider.

7.4 Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA)
